Our Support Coordinators are here to unravel the supports outlined in your NDIS plan, enabling you to independently access services that align with your needs.

The NDIS offers resources to support individuals living with disabilities in connecting with necessary services.
Our Support Coordinator collaborates with you to maximize the effectiveness of your NDIS plan. Together, you identify and coordinate service providers aligned with your goals, fostering skill development for increased independence and community connection. Additionally, your Support Coordinator can assist you with various tasks, including but not limited to:
- Clarify the workings of your plan and the available options
- Aid you in decision-making, exercising choice, and maintaining control over your plan
- Facilitate negotiations with providers regarding offered services, aligning with your goals and cost considerations
- Assist in budgeting for each required support type
- Coordinate support services and facilitate community access
- Organize any necessary assessments to qualify for funding
- Draft service agreements and bookings with your chosen providers
- Compile reports for the NDIS detailing your progress and any necessary adjustments
- Guide you in assessing your goals and preparing for your plan review
Eligibility:
The NDIS finances reasonable and necessary supports for individuals with disabilities. Since everyone’s needs and, consequently, their support plans are unique, not all individuals qualify for Support Coordination funding. To increase the likelihood of being eligible for Support Coordination, it is advisable to articulate the reasons you believe it is necessary during your planning meeting.
Potential eligibility factors for Support Coordination due to disability encompass challenges in communication, language barriers, complex support requirements, or limited social situations and support networks. Those who are incapable of making informed choices may also meet eligibility criteria.

These coaches specialize in assisting individuals diagnosed with psychosocial disabilities, ensuring access to specific supports that enrich their lives. Our coaching encompasses recovery and decision-making skills, working towards goals such as increased independence. We collaborate to develop personalized Recovery and Crisis Management Plans, drawing on our team’s expertise in mental health services.
